It combines the accessibility of a PPO network, the shopping tools of a consumer goods retailer and the investment options of a Health Savings Account (HSA), empowering you with the flexibility to make the healthcare decisions that are best for you and your family. This consumer directed health plan provides cost transparency and encourages smart consumer behavior by putting you in charge of your choices in care.

  • Choose the doctors you want to see – no referral needed to see a specialist.
  • Preventive care is covered at no cost to you.
  • You decide how and when to use the money in your HSA – pay for qualified expenses during the year, save it for future needs or invest for retirement.
  • You’re protected by an out-of-pocket maximum – once you meet the maximum, the plan plays 100% of covered expenses.
  • Take the HSA account with you when you leave the plan, change jobs or retire.
DENTAL (3 plans Available)

The CIGNA CDH Dental Plan is available in many PHH Mortgage locations. It operates like an HMO and is the most cost-effective plan. Your primary care dentist will coordinate all of your dental care. All of your preventive care is covered at 100% and all other care is paid according to a schedule of benefits.

The CIGNA PPO Dental Plan offers a directory of dentists who have agreed to provide their services at a preferred rate. You do not have to use PPO dentists exclusively; however, your out-of-pocket costs are lowest when you use a PPO network dentist. Preventive care is covered at 100% and other covered expenses are subject to deductibles and coinsurance.

The CIGNA Traditional Plan offers you the opportunity to choose any dentist you prefer. Preventive care is covered at 100% and is not subject to deductibles and coinsurance. All other covered expenses are subject to an annual deductible, a schedule of reimbursement and coinsurance.

 

VISION

The Vision Service Plan (VSP) covers a wide range of vision-related services. The in-network benefit is the most cost-effective with 100% coverage for most services. Benefits for eye exams and lenses are limited to one per year.
